(1) I/We agree the bank to charge from my/our given account(s) for any service charges of SMS Alert Service and I/We will ensure that there are enough funds in my/our accounts for such debit transactions regarding to SMS Alert Service charges.

(2) In the case of insufficient funds on the date of debiting from the account, the bank will charge (2) months accumulated service charges on the debit day of the following month. If the account has no sufficient funds for two consecutive months of debiting, SMS Alert Service will be terminated and the service charges will be debited from the account accordingly when the account has the sufficient funds. If this SMS Alert Service would like to be used again, I/We acknowledge that I/We have to apply for new SMS Alert Service. In the case of SMS Alert Service termination, I/We will inform the bank (1) month in advance.

(3) We agree that SMS Alert will only be received for the transactions of as per package type.

(4) I/We have read and agreed to abide by and be bound by the Terms and Conditions (a copy which will be given) governing AYA SMS Alert Service and any amendment set forth from time to time by the Bank. I/We understand that the mobile phone number provided for SMS Alert service will be used as the primary contact number of the given account regards on operating the banking services.

(5) If the applicant is the organization, the organization (company) has agreed to subscribe AYA SMS Alert service in accordance with the Board of director resolution dated.

(6) In case of the damage, change or loss of the mobile contact number that is subscribed to SMS Alert Service, the applicant must inform any AYA branch immediately. The applicant agrees to pay for any service fees occurred due to the damage, change or loss of the mobile contact numbers.

(၁) ယခု ဝန်ဆောင်မှုအတွက် ပေးချေရမည့် ဝန်ဆောင်ခများကို မိမိတို့ ဘဏ်စာရင်းထဲမှ ကောက်ခံ နိုင်ကြောင်း သဘောတူပါသည်။ ထိုသို့ကောက်ခံနိုင်ရန်အတွက်လည်း ငွေလုံလောက်စွာ ဖြည့်သွင်း ပေးထားမည် ဖြစ်ပါသည်။

(၂) အကယ်၍ ကောက်ခံရန် ငွေလုံလောက်မှု မရှိပါက ယခုလ ဝန်ဆောင်ခကို လာမည့်လတွင် ပေါင်း၍ စုစုပေါင်း (၂) လစာ ကောက်ခံသွားမည် ဖြစ်ပါသည်။ ထပ်မံ၍ (၂) လစာ ဝန်ဆောင်ခ ကောက်ခံရန် ငွေလုံလောက်မှု မရှိခဲ့ပါက SMS Alert Service အား အသုံးမပြုတော့ပါဟု ဘဏ်မှ ယူဆ၍ ရပ်ဆိုင်းလိုက်မည်ဖြစ်သော်လည်း ပေးရန်ကျန်ရှိသော ဝန်ဆောင်ခများအား ယင်းငွေစာရင်းတွင် ငွေပမာဏပြည့်မီသည့်အခါတွင် ကောက်ခံသွားမည်ဖြစ်ပါသည်။ SMS Alert Service ကို ထပ်မံ အသုံးပြုလိုပါက ပြန်လည်လျှောက်ထားရမည်ဖြစ်ပြီး အသုံးပြုမှု ရပ်ဆိုင်းလိုပါက ဘဏ်သို့(၁)လ ကြိုတင်အကြောင်းကြားရမည် ဖြစ်ပါသည်။

(၃) ရွေးချယ်ထားသော အမျိုးအစားပေါ်မူတည်၍ SMS Alert Service ကို ရရှိမည် ဖြစ်ကြောင်း သဘောတူပါသည်။

(၄) AYA SMS Alert Service နှင့်ပတ်သက်သော စည်းမျဉ်းစည်းကမ်းများကို သိရှိကြောင်းနှင့် ယင်းစည်းမျဉ်းစည်းကမ်းများကို ဧရာဝတီဘဏ်မှ အချိန်နှင့်အမျှ ပြင်ဆင်/ပြောင်းလဲနိုင်ကြောင်းကို လက်ခံ သဘောတူပါသည်။ မိမိတို့ ငွေစာရင်း၏ ဘဏ်ဝန်ဆောင်မှုနှင့်ဆိုင်သော ကိစ္စရပ်များ ပြုလုပ်ရန် လိုအပ်သော ဆက်သွယ်မှုအတွက် SMS Alert Service လက်ခံမည့် ဖုန်းနံပါတ်ကိုသာ ပထမဦးစားပေး အသုံးပြုသွားမည်ဖြစ်ကြောင်း သိရှိနားလည်ပါသည်။

(၅) လျှောက်ထားသူသည် အဖွဲ့အစည်း တစ်ခုခုမှ ဖြစ်ပါက ယင်းအဖွဲ့အစည်း ဒါရိုက်တာများ၏ သဘောတူညီကြောင်း နေ့စွဲ နှင့် ဆုံးဖြတ်ချက်ပါသော စာရွက်စာတမ်းနှင့်အတူ လျှောက်ထားရမည် ဖြစ်သည်။

(၆) SMS Alert Service လက်ခံမည့် ဖုန်းနံပါတ်သည် ပျက်စီး/ပျောက်ဆုံး/ ပြောင်းလဲခြင်းရှိပါက နီးစပ်ရာ ဧရာဝတီဘဏ်ခွဲများသို့ ချက်ချင်း အကြောင်းကြားရမည်ဖြစ်ပြီး ယင်းနှင့် ပတ်သက်၍ ဖြစ်ပေါ်လာသော မည်သည့် ဝန်ဆောင်ခများကိုမဆို လျှောက်ထားသူဘက်မှ ပေးဆောင်ရန် သဘောတူပါသည်။
